Luckily, "underwear" also includes socks (I really hate wearing shoes without socks) and long-johns, although I doubt they're going anywhere that cold. If the GM is kind, it could even include liners for helmets, which could add a little to defense against a slicing blow as well as making the player more comfortable.
I remember one wish that a party member got: she could "clean body" by saying "hot shower now". It actually improved reactions from npcs sometimes. The GM would roll a d6, and add +1 to reaction dice on a six. It also helped disease/infection resistance by +1, although that situation never actually arose.
